Cerebrospinal fluid (CSF) rhinorrhoea is a rare but serious condition characterized by the leakage of CSF from the subarachnoid space into the nasal cavity, usually due to a bony or dural defect at the skull base. Conventional management primarily involves surgical repair of the defect. Here, we present a remarkable case of CSF rhinorrhoea managed successfully without surgery through individualized homoeopathic treatment and transmission of medicine via hair.
Case History
The patient initially mistook his condition for a common cold. However, when the watery nasal discharge persisted, he consulted AIIMS Patna, where CT cisternography revealed:
Bony defect: ~6.5 mm at the inferolateral wall of the right sphenoid sinus.
Diagnosis: CSF rhinorrhoea.
The doctors strongly recommended surgical repair. Unsatisfied with surgical advice, the patient sought a second opinion at VMMC Safdarjung Hospital, New Delhi, where again surgery was advised along with a short course of medication (Tab. Diamox for one month).

The patient demonstrated live during the consultation how CSF leaked while stooping forward.
Dr. Ravi explained the hair transmission method of medicine. Initially hesitant, the patient took around 15 days to decide. Eventually, he sent his hair via speed post, and a detailed case history was taken.

Follow-Up Timeline
24 March 2025: Patient reported no CSF leak since 5 days. He expressed surprise at the improvement.
15 April 2025 (Zoom): PATIENT TOLD THAT THERE IS NO CSF HE IS BETTER NOW

28 May 2025 (Zoom): The patient talked with Dr Ravi over the zoom call on 28 May 2025. He further said that he did’t observed any CSF leak after 15th March. He was advised to avoid stooping and sneezing. He got advised for the MR Cisternography to rule out any pathology.
He said will do that.
6 August 2025 (Zoom): On 6th Aug 2025 Dr Ravi again talked with patient over the zoom call. As he reported that after 14th March. There is no csf leak. He was at his hometown and was again advised to undergo a MR cisternography. He promised for that soon. As per symptoms he was declared as CURED. Report of MR Cisternography awaited.
18 August 2025 (Report): “Both cribriform plates appear normal. No definite active CSF leak is seen.”
Dr Ravi talked with patient over the zoom video call on 24th Aug 2025. He is happy with the treatment outcome as he got cured about his disease CSF rhinorrhea. Cure was according to symptoms and also confirmed radiologically.
So we have decided to stop the treatment. Hair of the patient got detached from medicated vial and washed.
Outcome
Clinical Cure: No CSF leak since mid-March 2025.
Radiological Cure: MRI cisternography confirmed closure of the bony defect.
No Surgery: Patient avoided high-risk cranial surgery.
Method: Successful treatment with medicine transmission through hair.
Discussion
This case is unique because:
The patient had a 6.5–7 mm skull base defect with meningoencephalocele.
Conventional medicine only offered surgery, but the patient opted for homoeopathy.
Homoeopathic remedy administered via hair transmission method resulted in both symptomatic relief and radiological healing.
The cure was achieved remotely while the patient resided in Bihar and occasionally in Delhi, highlighting the feasibility of this unique method.
Conclusion
This case demonstrates that homoeopathy, through advanced transmission techniques, can achieve results even in conditions conventionally considered exclusively surgical.
Such cases call for further documentation and peer-reviewed research into the potential role of homoeopathy in managing CSF rhinorrhoea and other neurosurgical conditions.
