Hirschsprung’s disease is the most common cause of lower intestinal obstruction in neonates. Hirschsprung’s disease also known as aganglionic megacolon, is a congenital anomaly caused by migratory failure of neural crest cells. This leads to abnormal innervations of the bowel. Symptoms Signs and symptoms of Hirschsprung’s disease vary with the severity of the condition. Usually, signs and symptoms appear shortly after birth, but sometimes they’re not apparent until later in life. Typically, the most obvious sign is a newborn’s failure to have a bowel movement within 48 hours after birth. Other signs and symptoms in newborns may include: Swollen belly Vomiting, including vomiting a green or brown substance Constipation or gas, which might make a newborn fussy Diarrhoea Delayed passage of meconium — a newborn’s first bowel movement In older children, signs and symptoms can include: Swollen belly Chronic constipation Gas Failure to thrive Fatigue Causes It’s not clear what causes Hirschsprung’s disease. It sometimes occurs in families and might, in some cases, be associated with a genetic mutation. Hirschsprung’s disease occurs when nerve cells in the colon don’t form completely. Nerves in the colon control the muscle contractions that move food through the bowels. Without the contractions, stool stays in the large intestine. Having other inherited conditions. Hirschsprung’s disease is associated with certain inherited conditions, such as Down syndrome and other abnormalities present at birth, such as congenital heart disease. Complications Children who have Hirschsprung’s disease are prone to a serious intestinal infection called enterocolitis. Enterocolitis can be life-threatening and requires immediate treatment. DIAGNOSIS Radiology 1. Plain x-rays of the abdomen: Erect & supine 2. Contrast Enema. Shows narrow distal segment,funnel-shaped dilatation at level of transition zone with marked dilatation of the proximal colon. Rectal biopsy : Rectal biopsy is the definitive diagnostic test ( with inputs from Mayclinic ) Case Study This 20-day old child came to my clinic on 18 November 2021 after he was advised for surgery at KGMC. Father visited at the KGMC with a reference of his known as the child was not passing the stool. His abdomen was distended, and he gave birth very few times. The child cried all day and night; hence they visited King George Medical College. Dr advised for the barium X-ray, and on seeing the report, they recommended the surgical procedure. They said that a colostomy bag would be installed for a year, the only solution for this disease. However, they have not assured him that there will be 100 % recovery. Such cases are not frequently seen at Homoeopathic OPD, and I was also not well aware of this disease condition. This was my first case. I shared the report with one of my Radiologist friends Dr Dheraj, and he diagnosed the condition as Hirschsprung’s disease. When I asked him about the prognosis, he said POOR. Parents were not in the economic position for surgery and opted for my treatment. After a detailed case history, I have transmitted Opium 0/1 as a remedy. He made an incredible recovery from day one. The child’s parents visited the clinic on 14 December 2021 and reported about the condition. Now, the child cries less as it sees abdominal pain got reduced to the minimum. The stool is now formed; before the treatment, the stool was loose and green in colour. Parents visited along with the child on 14 February 2022. The child was doing excellent; the abdomen is now soft and no more distended. No pain in the abdomen, and the child is sleeping well.
